Bioidentical Testosterone Therapy (750$)

Service Description

This treatment will optimize your testosterone for 4-5 months. Only 1 visit for insertion of the pellet is needed. No more visits are required after the insertion of the pellet. The longevity (duration) of this therapy will be between 4 to 5 months. In order to receive this treatment, Blood labs must be provided. You can have your labs done at our office for a fee (Not included in the price of this treatment) ( We do not take Medical Insurance for labs) or you can have it done at any Lab of your convenience (Labcorp or Guest). If you need a requisition "Order" for the Labs we can provide one at no additional fee if you are receiving this service with us. We need to have the following lab results before receiving treatment ( Labs should not be older than 14 days): Estradiol Testosterone Total Testosterone Free PSA total ( ages 55-69 or high risk) TSH T3 T4 TPO (Thyroid Peroxidase) CBC Complete Metabolic Panel Vitamin D, 25- Hydroxy Vitamin B12 Lipid Panel Homocysteine A1C Reverse T3 Anti-Thyroglobulin antibody Prolactin

Cancellation Policy

We require a deposit of 103.50$ for any Filler/Botox appointment. There is NOT refund if the cancellation occurs the same day. You need to cancel or reschedule a minimum of 72 hours in advance to get refunded
