Only pick this option if you have been seen previously for Primary Care Services / General Medicine with us. (*If you have come to our practice only for Aesthetic procedures this does not apply to you. ) Only established Primary Care Patients with previously established medical history and medication record can use this option.
Cancellation Policy
We require a deposit of 103.50$ for any Filler/Botox appointment. There is NOT refund if the cancellation occurs the same day. You need to cancel or reschedule a minimum of 72 hours in advance to get refunded
